His work will address surgical gesture recognition for phase detection in the objective of improving the feedback to the surgeon.

&lt;strong&gt;ABSTRACT&lt;/strong&gt;
Deep brain stimulation of the medial globus pallidus is a surgical procedure for treating patients suffering from Parkinson’s disease. Its therapeutic effect may be limited by the presence of pyramidal tract side effect (PTSE). PTSE is a contraction time-locked to the stimulation when the current spreading reaches the motor fibers of the pyramidal tract within the internal capsule. The lack of side-effect predictive model leads the neurologist to secure an optimal electrode placement by iterating clinical testing on an awake patient during the surgical procedure. The objective of the study was to propose a preoperative predictive model of PTSE. A machine learning based method called PyMAN (for Pyramidal tract side effect Model based on Artificial Neural network) that accounted for the current of the stimulation, the 3D electrode coordinates and the angle of the trajectory, was designed to predict the occurence of PTSE. Ten patients implanted in the medial globus pallidus have been tested by a clinician to create a labeled dataset of the stimulation parameters that trigger PTSE. The kappa index value between the data predicted by PyMAN and the labeled data was .78. Further evaluation studies are desirable to confirm whether PyMAN could be a reliable tool for assisting the surgeon to prevent PTSE during the preoperative planning.

The importance of surgical simulation is now well admitted: for surgical education, assessment, or patient-based training. Creating realistic simulators should require complete understanding of surgical procedures and related processes. This involved access to detailed description of surgical processes. Numerous books and journals have described the major surgical procedures (such as surgical approaches or techniques of excision), complemented by a multitude of technical notes. However, and specially in neurosurgery, there are very few works on the description of procedures including both information available before the operation (predictive data) that is specific to patients, and the different actions constituting the procedure itself. From various sources of records in neurosurgical procedures, we established a method for modeling and analysing surgical procedures. Studies on cervical spine by anterior way (anterior cervical decompression and fusion: ACDF) were performed to propose models that better describe and explain such surgical procedures. The methodology is mainly based on the extraction of decision rules. We have also proposed a metric reflecting the diversity of the procedure investigated, allowing the comparison of different kinds of populations.

&lt;strong&gt;ABSTRACT&lt;/strong&gt; &lt;br/&gt;

Purpose - With the intention of extending the perception and action of surgical staff inside the operating room, the medical community has expressed a growing interest towards context-aware systems. Requiring an accurate identification of the surgical workflow, such systems make use of data from a diverse set of available sensors. In this paper, we propose a fully data-driven and real-time method for segmentation and recognition of surgical phases using a combination of video data and instrument usage signals, exploiting no prior knowledge. We also introduce new validation metrics for assessment of workflow detection. &lt;br/&gt;

Methods - The segmentation and recognition are based on a four-stage process. Firstly, during the learning time, a Surgical Process Model is automatically constructed from data annotations to guide the following process. Secondly, data samples are described using a combination of low-level visual cues and instrument information. Then, in the third stage, these descriptions are employed to train a set of AdaBoost classifiers capable of distinguishing one surgical phase from others. Finally, AdaBoost responses are used as input to a Hidden semi-Markov Model in order to obtain a final decision. &lt;br/&gt;

Results - On the MICCAI EndoVis challenge laparoscopic dataset we achieved a precision and a recall of 91 % in classification of 7 phases. &lt;br/&gt;

Conclusion - Compared to the analysis based on one data type only, a combination of visual features and instrument signals allows better segmentation, reduction of the detection delay and discovery of the correct phase order.

Although gross total resection in low-grade glioma surgery leads to a better patient outcome, the
in-vivo control of resection borders remains challenging. For this purpose, navigated beta-probe
systems combined with FDG radiotracer, relying on activity distribution surface estimation, have
been proposed to generate reconstructed images. The clinical relevance has been outlined by
early studies where intraoperative functional information are leveraged although inducing low
spatial resolution in reconstruction. To improve reconstruction quality, acquisition models have
been suggested. They involve the definition of attenuation matrix for designing radiation
detection physics. Yet, they require high computational power for an efficient intraoperative
usage. To address the problem, we propose a new acquisition model called Partition Model (PM)
considering an existing model where coefficients of the matrix are taken from a look-up table
(LUT). Our model is based upon the division of the LUT into particular homogeneous values for
assigning attenuation coefficients. To validate our model, we ran multiple experiments using in-
vitro data sets. Three spatial configurations of grown cancer cells using Petri dish as phantom,
simulating tumors and peri-tumoral tissues, have been acquired. We compared our acquisition
model with the off-the-shelf LUT and the raw method. Acquisition models outperformed the raw
method in term of tumor contrast (7.97:1 mean T:B) but with an impossibility of real time usage.
Both acquisition models reached the same detection performances with references (0.8 mean
AUC and 0.77 mean NCC). Yet PM slightly improves the mean tumor contrast up to 10.1:1 vs
9.9:1 with the LUT model and more importantly reduces the mean computation time by 7.5%.

&lt;strong&gt;ABSTRACT&lt;/strong&gt;
Deep brain stimulation of the medial globus pallidus is a surgical procedure for treating patients suffering from Parkinson’s disease. Its therapeutic effect may be limited by the presence of pyramidal tract side effect (PTSE). PTSE is a contraction time-locked to the stimulation when the current spreading reaches the motor fibers of the pyramidal tract within the internal capsule. The lack of side-effect predictive model leads the neurologist to secure an optimal electrode placement by iterating clinical testing on an awake patient during the surgical procedure. The objective of the study was to propose a preoperative predictive model of PTSE. A machine learning based method called PyMAN (for Pyramidal tract side effect Model based on Artificial Neural network) that accounted for the current of the stimulation, the 3D electrode coordinates and the angle of the trajectory, was designed to predict the occurence of PTSE. Ten patients implanted in the medial globus pallidus have been tested by a clinician to create a labeled dataset of the stimulation parameters that trigger PTSE. The kappa index value between the data predicted by PyMAN and the labeled data was .78. Further evaluation studies are desirable to confirm whether PyMAN could be a reliable tool for assisting the surgeon to prevent PTSE during the preoperative planning.

Analysis, recognition and execution of surgical gestures for surgical robotic training
&lt;/p&gt;

&lt;strong&gt;Abstract&lt;/strong&gt;&lt;br/&gt;

Integration of robotic systems in the operating room changed the way that surgeries are performed. It modifies practices to improve medical benefits for the patient but also brought non-traditional aspects that can lead to serious adverse events. Recent studies from the French authorities for hygiene and medical care highlight that these events mainly come from surgeons&amp;#039; technical skills, which question surgical robotic training and teaching. To overcome this issue, surgical simulators help to train practitioner through different training tasks and provide feedback to the operator. However the feedback is partial and do not help the surgeon to understand gestural mistakes. Thus, we want to improve the surgical robotic training conditions. The objective of this work is twofold. First, we developed a new method for segmentation and recognition of surgical gestures during training sessions based on an unsupervised approach. From surgical tools kinematic data, we are able to achieve gesture recognition at 82%. Thismethod is a first step to evaluate technical skills based on gestures and not only on the global execution of the task as it is done nowadays. The second objective is to provide easier access to surgical training and make it cheaper. To do so, we studied a new contactless human-machine interface to control surgical robots. In this work, the interface is plugged to a Raven-II robot dedicated to surgical robotics research. Then, we evaluated the performance of such system through multiple studies, concluding that this interface can be used to control surgical robots. In the end, one can consider to use this contactless interface for surgical training with a simulator. It
can reduce the training cost and also improve the access for novice surgeons to technical skills training dedicated to surgical robotics.
